Medication related hospital admission is a significant burden on Australia’s healthcare system, costing the taxpayers hundreds of millions of dollars per year. A substantial proportion of these medication related hospital admissions are preventable through better medication management. A home medicines review (HMR) is an assessment that occurs between an accredited pharmacist and a patient within the patient’s home. The interview focuses on the drug/medication therapy for the patient and the aim is to maximise the potential benefits of the medications and minimise adverse effects. Other aims of a HMR include increasing patient and GP knowledge of medications.
A HMR will often involve general health and medication education to help patients improve their overall quality of life and benefits from their medications.
A referral from your GP is required to have a HMR initiated. Your accredited pharmacist then performs the HMR interview, and a full report is then delivered to your GP who will discuss the findings of the HMR at a subsequent appointment.
To arrange a HMR for yourself or a loved one, please speak with one of our receptionists who will be able to advise on the procedures involved in co-ordinating a visit by our Pharmacist, Fabian Bucco in conjunction with your treating doctor.
A home based Sleep Apnoea testing service and CPAP therapy service is offered via Prospect Pharmacy Advice for this increasingly prevalent and important medical condition. A referral may be required from your GP to utilise the service. Home based sleep testing is coordinated by the pharmacy with results sent promptly back to your referring GP. If recommendations are made to commence a trial of CPAP therapy, our trained pharmacy staff are able to initiate and supervise your CPAP therapy.
The pharmacy can take the hassle out of medication management by preparing weekly blister packs, also known as Webster Packs or Medico Packs.
If you have trouble leaving the house, we are willing to help. Ask about free local delivery.
Avoid misplacing prescriptions by keeping them on file at the pharmacy. This service works particularly well with the local delivery service.
All subsidised Diabetes supplies are available at the pharmacy.
